Job Description:Working with physicians and oncology service
line staff, the Nurse Navigator/Patient Educator / Program
Coordinator is responsible for developing, facilitating and/or
coordinating system-wide patient care services developed for
specific malignancies. The Nurse Navigator /Patient Educator will
function as a patient advocate and resource, program
coordinator/developer, clinical educator, and program marketing
expertise. The Nurse Navigator /Patient Educator will develop and
maintain a high degree of expertise and knowledge regarding state
of the art technology and services related to Oncology and adhere
to the Health System mission in providing quality care service.
Provides oversight and coordination of the Bariatric Program at the
Concord Medical Center.Education:
