Shift Notes 12*3,Days Start- March and April starts available
AANP Certified - REQUIRED Active CA license - REQUIRED Active ACLS,
BLS, DEA, PECOS - REQUIRED for credentialing Must disclose provider
availability (# of shifts per month) - REQUIRED Must be available
for the length of assignment and dates - REQUIRED Clean
Malpractice/Background - REQUIRED Disclose if provider has worked
at any other CHI/Dignity facility - REQUIRED COVID VACCINATION -
REQUIRED Specialty: Pre and Post Cardiac Electrophysiology patients
Setting: Telemetry and Pre/Post Cardiac Unit Employment Type: Full
Time Salary: $110 - $130 Hourly Bonus/Commission: No
